Betamethasone valerate/minoxidil/
tretinoin
A 28-year-old woman developed kerion celsi during
topical treatment with betamethasone valerate, minoxidil
and tretinoin for alopecia areata.
The woman, who had recurrent alopecia areata,
presented with a scaly, purulent granuloma in the frontal
region. Four months earlier, she received a 40-day course
of topical minoxidil 2%, tretinoin lotion 0.05% and
betamethasone valerate 0.1% [dosage not clearly stated].
Microscopic examination of the hair and scales revealed
spore sheaths surrounding the hair shafts. Culture was
positive for Microsporum canis. She was diagnosed with
tinea microsporica granulomatosa.
Recovery was complete after a course of systemic
itraconazole and topical Tincture rubra of Castellani.
Author comment: "[W]e hypothesize that the drug
regimen used for alopecia areata (Minoxidil [2%]+tretinoin
lotion [0.05%]+betamethasone valerate [0.1%]), promoted
the Microsporum canis infection."
